Make a Claim

 

Making a claim is simple with ShelterCare, just follow the steps below:
  1. Download and print out the .PDF of the claim form. Download Claim Form Here

  2. Fill out Part 1 of the form. Be sure to print clearly and provide all required information.

  3. Take the form to your veterinarian and have them fill out Part 2 of the form and sign.
    Note: Ask your veterinarian if they are using EVE (real-time claims filing for veterinary hospitals and clinics.)

  4. Sign and date your claim form before you fax or mail it in.

I decided to sign up for pet insurance with PetCare after a friend told me about it, and I realized it wasn't a bad price, and that I could afford it!
